Job Title: Assistant Case Manager
Job Summary
This is a paraprofessional position that performs a variety of case management duties for Medicare/Commercial Advantage Plans. Job functions include coordinating medical appointments and providing managed care agencies with clinical updates as requested.
Essential Job Functions
• Review all Medicare Advantage (Managed Care/HMO) admissions and discharges daily.
• Assist case manager in coordinating follow up medical appointments
• Review all pending medical appointments and follow up with nursing units and transportation department.
• Provide clinical updates and other documentation requested by Managed Care agencies.
• Coordinate equipment and services for pending discharges.
• Provide bed management oversight, and other duties as needed
Job Requirements
Education:
• High School Diploma or equivalent. Associate degree preferred.
Experience:
• One-year experience working in a community service agency that provides service to elderly clients preferred
Licenses/Certifications:
• N/A
Abilities Required
• Sensitivity and skill required in working with impaired elders and families by phone
• Must be organized, able to work independently and understand confidentiality which is necessary for this program.
• Must be bi-lingual (English/Spanish)
• Good communication skills
