Work Environment

This position offers the flexibility of remote work, allowing you to effectively engage with clients and families from the comfort of your own home while still being integral to our hospice team. Key Responsibilities
  • Conduct thorough psychosocial assessments of clients, taking into account emotional, mental, and social factors affecting their well-being.
  • Develop individualized care plans in collaboration with clients and their families, outlining achievable goals and identifying necessary services and supports.
  • Provide counseling and emotional support, guiding clients through life transitions, such as end-of-life decisions and family dynamics.
  • Advocate for clientsโ€™ rights, ensuring access to essential services and community resources tailored to their specific needs.
  • Work cohesively with nurses, caregivers, and other healthcare professionals to establish communication channels that promote client-centered care.
  • Regularly visit clients to monitor their well-being and adjust care plans as necessary, addressing safety concerns and adapting to changes in their condition.
  • Facilitate family meetings to improve communication and resolve conflicts among family members about care and treatment.

Qualifications
