• Conduct thorough clinical assessments of patients referred to the hospice program, considering physical, emotional, and social factors affecting their care.
• Develop and implement individualized treatment plans that address the unique needs of each patient and their family, utilizing evidence-based practice approaches.
• Administer therapeutic interventions, including solution-focused therapy and bereavement counseling, to assist patients and families in coping during end-of-life transitions.
• Maintain precise and detailed clinical records, documenting interventions and patient progress to ensure compliance with healthcare regulations.
• Collaborate with a multidisciplinary team, including nurses, physicians, and chaplains, to provide integrated support and care for each patient’s unique circumstances.
• Empower patients and family members through education about resources, coping strategies, and mental health solutions, facilitating a deeper understanding of their emotional journey.
• Stay informed about the latest advancements in hospice care and mental health practices to enhance service delivery and personal professional development.

In this role, you will engage primarily with patients who are facing terminal diagnoses and their families, addressing the psychological, emotional, and social challenges that arise. Your expertise will help facilitate open communication about end-of-life concerns, grief, and adjustment, enhancing the overall quality of their experience during hospice care.

• A Master’s degree in Social Work from an accredited institution.
• Current and valid LCSW license in the state of Oregon.
• Demonstrated knowledge of solution-focused therapies and techniques relevant to end-of-life care.
• Strong communication skills, with the ability to empathize and provide guidance during difficult conversations.
• Proficiency in electronic health records systems and good record-keeping practices.

• A minimum of two years of experience in clinical social work, particularly in the hospice or palliative care setting.
• Experience working with diverse populations, with a sensitivity to cultural considerations in end-of-life care.
• Familiarity with hospice regulations and interdisciplinary collaboration in a health care environment.
