Key Responsibilities:

  • Assess, plan, implement, and evaluate nursing care for a diverse patient population with medical diagnoses (e.g., pneumonia, CHF, COPD, diabetes) and post-surgical conditions (e.g., appendectomy, cholecystectomy, orthopedic repairs, abdominal surgeries).
  • Administer medications, fluids, and blood products as ordered, monitoring for adverse reactions.
  • Perform frequent patient assessments, including vital signs, pain levels, intake/output, and mental status.
  • Collaborate with physicians, case managers, physical therapists, and other healthcare team members to coordinate discharge planning and continuity of care.
  • Provide patient and family education regarding diagnoses, medications, post-discharge instructions, and disease management.
  • Monitor for post-operative complications including infection, bleeding, deep vein thrombosis (DVT), pulmonary embolism (PE), and ileus.
  • Assist patients with activities of daily living (ADLs) such as bathing, ambulation, toileting, and feeding as needed.
  • Respond to changes in patient condition, initiate appropriate interventions, and communicate concerns to the medical team.
  • Maintain accurate, timely documentation of all nursing care and patient responses in the EMR.
  • Ensure a safe, clean, and organized patient environment, including fall prevention protocols and infection control measures.

Qualifications:

  • Education: Completion of an accredited Registered Nurse (RN) program (Associate's or Bachelor's degree).
