Job Summary:

We are seeking a skilled and dedicated Registered Nurse (RN) to join our team and provide professional nursing care to patients in a home health setting. The RN will work closely with patients, families, and interdisciplinary healthcare teams to develop and implement personalized care plans, monitor patient progress, and ensure the highest level of care is delivered.

Responsibilities:

Conduct in-home patient assessments and develop individualized care plans.

Provide skilled nursing care, including wound care, medication management, IV therapy, and injections.

Monitor patient conditions, document changes, and communicate concerns to physicians and care teams.

Educate patients and caregivers on disease management, medication adherence, and lifestyle modifications.

Collaborate with physical therapists, occupational therapists, social workers, and home health aides to coordinate care.

Ensure compliance with all state, federal, and agency regulations and protocols.

Maintain accurate and up-to-date patient records and documentation.

Provide emotional support and guidance to patients and their families.

Qualifications:

Active RN license in Indiana.
