Job Title: Home Infusion Registered Nurse

Job Description

This role provides high-quality home infusion nursing care by coordinating and managing all aspects of infusion therapy as ordered by the physician. The nurse uses the full nursing process and systematic critical thinking to assess, plan, implement, and evaluate care in collaboration with patients, their families, and the broader health care team. The position supports patients across a variety of clinical conditions, promotes and maintains health through education, and ensures timely and accurate documentation in compliance with all regulatory and confidentiality requirements.

Responsibilities

  • Use the nursing process to assess, plan, implement, and evaluate infusion therapy and related nursing care for patients in home, hospital, or clinical settings.

  • Apply systematic critical-thinking skills to guide data gathering, patient assessment, and sound nursing judgment in all aspects of care.

  • Coordinate and manage all aspects of infusion nursing care as ordered by the physician, including preparation, administration, monitoring, and follow-up.

  • Deliver patient-centered care in collaboration with the patient, their family, and all members of the interdisciplinary health care team.

  • Complete and submit required documentation within established time frames, including initial assessments, visit encounters, and patient education records.

  • Participate in interdisciplinary collaboration and communicate urgent or significant clinical findings promptly to the appropriate providers and team members.

  • Maintain required annual competencies and stay current with best practices in infusion therapy and home health nursing.

  • Protect the privacy, security, and confidentiality of patient, staff, and organizational data, adhering to all applicable federal and state regulations, including HIPAA.

  • Foster a non-discriminatory, culturally sensitive care environment that respects the sociocultural needs and preferences of patients and families.

  • Work independently with minimal supervision, demonstrating strong work ethic, accountability, and excellent customer service to patients and families.

  • Travel throughout the assigned territory as needed, adapting schedules and routes to meet patient needs.

  • Provide availability and support for on-call responsibilities as needed, including responding to patient needs outside of standard business hours.

  • Maintain a neat, clean, and professional appearance and demeanor at all times while representing the organization in the community.

  • Safeguard company proprietary information and uphold complete confidentiality regarding patient care and all forms of protected health information (PHI and ePHI).

  • Demonstrate excellent IV skills, including starting and maintaining peripheral and central lines, administering medications, and monitoring for complications.

  • Safely handle and transport necessary equipment and supplies, including lifting items up to 10 pounds as required for patient care.

  • Operate a motor vehicle safely, including in adverse weather conditions, to reach patient homes or clinical sites as assigned.

Essential Skills

  • Current, active registered nurse license in good standing in the state of employment.

  • Experience administering infusion therapy in a home, hospital, or clinical setting.

  • Strong infusion therapy knowledge and hands-on skills, including IV insertion and management.

  • Excellent IV skills for starting and maintaining lines and administering medications.

  • Current CPR certification.

  • Ability to apply systematic critical-thinking and sound nursing judgment in complex clinical situations.

  • Ability to work independently with minimal supervision while maintaining high standards of care.

  • Strong documentation skills and ability to complete assessments, visit notes, and patient education records accurately and on time.

  • Ability to maintain confidentiality and comply with all federal and state laws related to protected health information (PHI and ePHI), including HIPAA guidelines.

  • Valid driver's license and ability to operate a motor vehicle, including in adverse weather conditions.

  • Ability to lift at least 10 pounds as needed for equipment and supplies.

  • Strong communication and collaboration skills for working with patients, families, and interdisciplinary health care teams.

Additional Skills & Qualifications

  • Three to five years of experience in homecare is preferred.

  • Experience in ER, ICU, GI units, home health, medical-surgical, or oncology nursing is highly valued.

  • Experience in acute care, oncology, or other high-acuity settings is a plus.

  • Pediatric, ER, and/or medical-surgical experience is considered a plus.

  • Experience with infusion therapy in home infusion programs or infusion centers is beneficial.

  • Familiarity with patient care in diverse settings, including home, hospital, and outpatient clinics.

  • Experience administering injections and managing complex medication regimens.

  • Comfort working with healthcare technology platforms such as WeInfuse and AlayaCare, or similar systems, is advantageous.

  • Demonstrated commitment to providing stellar customer service and a positive patient experience.

Work Environment

This position supports infusion therapy in home, hospital, and clinical settings, with one role focused on a full-time infusion center and another on field-based home infusion nursing. The nurse frequently travels within an assigned territory to provide care in patients' homes and may also work in an infusion center environment. Work hours may include participation in an on-call rotation to support patients outside of standard business hours. The role involves regular use of infusion equipment, IV supplies, and electronic health record or clinical software systems such as WeInfuse and AlayaCare. The nurse must be comfortable driving in various weather conditions and transporting light equipment and supplies. A neat, clean, and professional appearance and demeanor is expected at all times while interacting with patients, families, and health care team members. Pay and Benefits

The pay range for this position is $89440.00 - $97760.00/yr.

Individual compensation offered for this position within this range will depend on many factors, including qualifications, skills, relevant experience, job knowledge, geographic location, internal equity, and other pertinent job-related factors.

Standard Health Benefits, PTO, Mileage reimbursement or company car and gas card provided (if driving ~1800 miles/month)
