Job description

Job Overview
Fairview Range is hiring a Registered Nurse (RN) Case Manager to join our Home Care and Hospice Team in Hibbing, MN.
The Case Manager in Home Care and Hospice plays a critical role in coordinating and delivering high-quality, patient-centered care to individuals receiving services in their homes or in hospice settings. This position is responsible for managing a caseload of patients, developing individualized care plans, overseeing implementation of clinical services, and ensuring that care is provided in accordance with medical standards, agency policies, and regulatory requirements. Serving as the primary point of contact for patients, families, and interdisciplinary care teams, the Case Manager facilitates communication, monitors patient progress, and adjusts care plans as needed to ensure comfort, dignity, and optimal outcomes. Compassion, clinical expertise, and strong organizational skills are essential in this role.
We consider it an honor to deliver care for our patients and their families and serve communities across the Iron Range. The RN Case Manager coordinates the delivery of all the care/services appropriate to the needs of the patients of all ages and their families using an interdisciplinary team approach. An individualized plan of care with specific goals is developed for each patient in collaboration with the physician.
Schedule: This position will work 72-80 hours per two weeks. The shift length will be 8-hours on the day shifts with rotating weekends and on call. The position works approximately 10 weekends per year, and 2-3 days of call each month.

As an RN Case Manager, you will be responsible for:
  • Conducting initial and ongoing assessments to determine patient needs and eligibility for home health or hospice services.
  • Developing, implementing, and regularly updating individualized care plans in collaboration with providers and interdisciplinary team members.
  • Identifying expected outcomes which are realistic, measurable, and attainable.
  • Collaborating with the interdisciplinary team to develop an appropriate discharge plan.

  • Coordinating delivery of nursing, therapy, social work, and other support services.
  • Providing age-specific direct patient care in collaboration with all disciplines according to standards of care and policy.
  • Serving as a liaison between patients, families, healthcare providers, and community resources.
  • Facilitating use of supplies and equipment in a cost-effective manner.
  • Ensuring documentation is accurate, timely, and compliant with Medicare/Medicaid, State, TJC and agency standards.
  • Monitoring patient progress, symptoms, and response to treatment; revise care plans accordingly.
  • Recognizing and reports changes in patient status to the patient's provider in a timely manner.
  • Assessing the patient's response to treatment on identified outcome and revises the plan of care accordingly.
  • Identifying patient/caregiver preferred method of learning and teaches according to patient/caregiver needs.
  • Providing education to patients/caregivers regarding disease processes, medications, home safety and end-of-life care.
  • Identifies other educational needs and alternative resources for education, if necessary.

Required Qualifications
  • Graduate of an accredited RN program
  • 1 year acute or long-term care nursing experience
  • Basic Life Support (American Heart Assoc or Red Cross) within 90 Days
  • MN Registered Nurse (RN) License
  • Current MN driver's license and auto insurance

Preferred Qualifications
  • Bachelor of Science Nursing with Public Health Nurse Certificate
  • 1 year home care experience
  • 1 year hospice experience
