Job description

The Heart Failure Integrated Practice Unit (IPU) serves as the foundation of a nationally recognized heart failure program, grounded in clinical excellence, innovation, and patient-centered care. In 2020, our program distinguished itself as the third in the nation to achieve Heart Failure Certification through DNV, reflecting commitment to evidence-based standards and a culture of continuous improvement. In 2022, we further demonstrated our commitment to exceptional outcomes by achieving Gold Plus Status from the American Heart Association, a designation we continue to maintain through sustained performance in quality metrics and guideline-directed care. 

Our multidisciplinary team plays a critical role in advancing patient outcomes through comprehensive education, individualized transition planning, and long-term disease management. These efforts directly impact reduced readmissions, improved symptom control, and enhanced quality of life for patients living with heart failure. The heart failure nurse is central to this work, providing expert nursing care by utilizing the nursing process to assess, plan, implement, and evaluate patient-centered care across the continuum. 

Practicing within the framework of organizational policies and procedures, the heart failure nurse demonstrates professional accountability, clinical leadership, and a commitment to ongoing growth. This role is responsible for upholding standards of nursing practice, coordinating patient care activities, and collaborating with the interdisciplinary team to ensure safe, effective, and high-quality care. Through clinical expertise, coordination of care, and patient advocacy, the heart failure nurse contributes significantly to the success of the program and the delivery of exceptional outcomes for our patients.

MINIMUM EDUCATION REQUIRED:
Graduate of a nursing program

MINIMUM EXPERIENCE REQUIRED:
New Graduates of a nursing program eligible

MINIMUM LICENSURE/CERTIFICATION REQUIRED BY LAW:
Current License in the State of Georgia as a Registered Nurse or NLC/eNLC Multistate License. BLS certification required.

ADDITIONAL QUALIFICATIONS:
Advanced certification in field of specialty, if applicable. Bachelor's degree preferred. Nursing Experience in Hospital Setting Preferred.
