Job description

Primary Care in the Rochester region is seeking a Nurse Practitioner or Physician Assistant to work as part of a multidisciplinary team to provide care to those living within our community through the Senior Service Home Visiting Programs. Within in this position the NP/PA will work collaboratively with internal medicine physicians to provide holistic care including acute, chronic, preventive and palliative care services to patients in private homes and senior living centers. The provision of care includes but is not limited to comprehensive evaluation, ordering and interpreting diagnostic tests, prescription of pharmacologic agents and treatments and non-pharmacologic therapies, and counseling patients. Ideal candidates have experience in Palliative Care, Primary care and/or Geriatrics.

During the OnDemand interview, a question will appear on your screen, and you will have time to consider each question before responding. You will have the opportunity to re-record your answer to each question - Mayo Clinic will only see the final recording. Nurse Practitioner: Masters-prepared graduate of an accredited school of nursing and Nurse Practitioner program. Current MN RN and APRN license and certification as a FNP, ANP or ACNP.  


Physician Assistant: Graduate of accredited Physician Assistant program with current PA certification by NCCPA and registration as a PA by the MN State Board of Medical Practice.  


Additional Qualifications: 
Active BLS and ACLS certification from American Heart Association or American Red Cross at start date and DEA registration required
