Medical Student Clinicals and Internships

Students interested in completing a medical student clinical rotation or internship with Livingston HealthCare must submit this application. To expedite the process, it is recommended to upload required documents with your application. These include:

  • Student Certification Form

  • CV/Resume

  • Rotation Objectives from Program

  • Letter of Good Standing from School/Program

  • Head Shot picture

  • Photo ID

  • Copy of Immunizations (2-step PPD)

  • Flu Shot Information
