Mental Health and/or ID/DD experience is preferred for this position.
Responsibilities and Duties for LPN:
  • Implementing and maintaining Individual Service Plans.
  • Conducting ongoing physical and mental assessments.
  • Maintaining accurate consumer records, monitoring consumer safety.
  • Supervising and monitoring individual medical treatment programs.
  • Administration and delivery of consumers’ prescribed medication as needed.
  • Monitoring and education regarding medication compliance and side effects.
  • Assessing consumer’s vital signs, height, weight, medication compliance, possible side effects, and obtaining AIMS score or laboratory tests as ordered by a physician.
  • Completing PA (prior authorizations) and patient assistance for consumer’s psychiatric medication needs.
  • Compliance with agency policies and procedures, and adherence to all relevant local, state, and federal legal and regulatory requirements.

Qualifications and Skills for LPN:
  • Graduation from an accredited Licensed Practical Nurse Program.
  • Current Virginia unrestricted LPN License or Current Unrestricted LPN License from a Nurse Licensure Compact State.
  • 1-year LPN work experience in a mental health program and/or residential setting.
  • 1-Year LPN work experience involving individuals with intellectual disabilities.
  • Valid driver’s license with acceptable DMV record.
  • Proficiency in Microsoft Office.
  • Preferred: Experience using Electronic medical records; basic knowledge of Medicaid/Medicare Regulations.
