Work Environment

This role is designed for a remote work environment, where the Social Worker will engage with clients primarily through virtual counseling and support sessions. Key Responsibilities

• Conduct thorough evaluations of psychosocial needs for clients, considering their emotional, mental, and social factors.
• Develop individualized care plans in collaboration with clients and their families, tailoring strategies to address specific goals.
• Provide counseling and emotional support, assisting seniors and their families in navigating significant life transitions, such as health changes or caregiving challenges.
• Advocate for client rights and independently assess their access to essential community resources and services.
• Collaborate with interdisciplinary teams, including healthcare professionals and volunteers, to enhance and streamline care delivered to clients.
• Regularly monitor clients’ well-being through check-in sessions, assessing their living conditions, and addressing any concerns that may arise.
• Facilitate constructive communication and mediate conflicts among family members involved in client care.
• Educate clients and families about available community resources, support groups, and social engagement opportunities.
• Maintain meticulous records, ensuring accurate documentation of client assessments, progress notes, and care strategies.
• Stay current with social work practices and ethical standards, ensuring compliance with state regulations.

Qualifications

Compensation and Benefits

This position offers a competitive salary ranging from $60,000 to $100,000 per year, commensurate with experience.
