ABOUT US:
The Lundquist Institute for Biomedical Innovation at Harbor-UCLA Medical Center is an independent, non-profit biomedical research organization located in Torrance, California. In dozens of laboratories throughout the campus, researchers are working to unravel the mysteries of heart and kidney disease, cardiac complications in newborns, chronic lung disease, skin cancer, sickle cell disease, disorders of the autoimmune system, and much more. Many of these projects are collaborative in nature, involving experts from different areas of specialization. Our discoveries have prevented blindness in newborns, enabled premature infants with fragile lungs to breathe and allowed children with rare, often fatal diseases to grow up healthy and strong.
Licensed Clinical Social Worker Program Coordinator
The LCSW Program Coordinator will perform professional social work service to patients and families with complex medical and related psychosocial needs in the ambulatory care setting while supervising and supporting MFP MSW social workers and MFP MSW student interns delivering team-based intervention services through the Medical-Financial Partnership Program.
The LCSW Program Coordinator position coordinates and facilitates services delivered by the Medical-Financial Partnership Program - a two-generation economic and social support intervention for families -- in prenatal, perinatal, pediatric, and family medicine care clinical settings to:
1) implement Medical-Financial Partnership program activities
2) ensure continuity and coordination of services
3) optimize service delivery
4) minimize gaps in care for families
5) supervise more junior MSW team members and trainees
6) develop and strengthen key care team and community partnership relations, communications, and care linkages.
  • The LCSW Program Coordinator oversees and supports Medical-Financial Partnership services across care settings and departments.
  • The LCSW Program Coordinator will provide direct clinical support for the Medical-Financial Partnership interdisciplinary program to promote relationship-centered care for expecting parents and parents of young children, including for those with high psychosocial and health risk. The Program Coordinator will collaborate with others in the broader interdisciplinary health care team and promote prenatal, perinatal, and pediatric parent-child attachment and early relational health through delivery of services that address structural, economic, and social drivers of behavioral, mental, and overall health. The LCSW Program Coordinator also assists in the collection of relevant clinical measures for patient care, program support, and evaluation.
  • The LCSW Program Coordinator will provide daily clinical direct service in collaboration with the Medical-Financial Partnership team, and ongoing education and supervision of social work interns exercised in accordance with scope of practice guidelines. These activities include timely, complete, and accurate documentation and administrative processes related to educational roles, in coordination with other program staff and leadership.
  • The LCSW Program Coordinator may participate in the recruitment and placement of social work interns and maintains relationships with schools of social work to facilitate intern placement processes.
  • The LCSW Program Coordinator will supports facilitation of Medical-Financial Partnership multi-disciplinary team conferences to discuss and evaluate clients' progress, and to develop care coordination and care transition plans.
  • The LCSW Program Coordinator will promote and maintain collaborative relationships to support service connections with various health care team members, including other social workers and providers in Los Angeles Department of Health Services Clinics where the Medical-Financial Partnership Program operates, and with service partner organizations and agencies in the community.

CLASSIFICATION STANDARDS:
  • Positions allocable to this class are assigned to ambulatory care setting and are characterized primarily by the highly specialized and difficult case assignments where the nature and extent of the patient's needs has a significant impact on the functioning and lifestyle of patient and family. Incumbents in these positions are expected to function with minimal technical supervision as they provide clinical individualized treatment to accomplish changes in insight attitudes, and motivation of patients and family members to attain optimal functioning.
  • At the level, providing professional social work services to patients with significant psychosocial needs is not an incidental part of the workload, but is regularly performed with infrequent recourse to supervisory guidance. Social work practice at this level demands highly professional skills and will include additional expertise in specific therapeutic methods such as motivational interviewing, brief cognitive behavioral therapeutic techniques, and narrative therapy. All position in this class provide consultation services to other health disciplines and community agencies about issues related to patient care and assessing unmet needs of the patient.
  • Incumbents in these positions may act as team leaders and may coordinate social work functions across multiple services by providing professional diagnostic and goal setting functions for the team members and by managing the system of case assignment. The evaluation, intervention planning, and resolution of issues may not be directly patient related. Positions within this class may be responsible for developing and coordinating social work services with other programs, formulating goals and objectives, and adapting policies and procedures to provide more effective social work services to patients and families.
  • Incumbents must possess knowledge of how to complete culturally and age appropriate psychosocial and emotional assessments, formulate strengths-based care plans, apply the appropriate trauma informed and evidence-based approaches, and implement care coordination to provide intervention for a wide range of care and service needs.

Clinical quality improvement activities to enhance team effectiveness
• Keeps informed in the fields of childhood social drivers of health, early relational health, perinatal mental health, Trauma-Informed Care, evidence-based practices, integrated health care models and practices, quality improvement, law and ethics in social work, Los Angeles County and local communities' resource landscape, as they are applicable to the populations served.
• Participates in Medical-Financial Partnership multi-disciplinary team conferences to discuss and evaluate client's progress, and to develop care coordination and care transition plans.
Essential Job Functions
  • Interviews individuals, their families and/or significant others to obtain a family, social, employment/financial, medical, substance abuse, and mental health treatment history. Assessment may also include parenting and child developmental information.
  • Collaborates with client/parent/family to develop a tentative care plan based on their goals designed to enhance client's capacity in addressing child/family adversity and related economic, social, and health related stressors, which may include referral to another resilience bridge team member for individual counseling, case management, and/or referral to another community-based organization.
  • Confers with representatives of other departments and community agencies to collaborate, troubleshoot, and to develop and maintain strong working relationships.
  • Participates as a member of administrative, educational, professional, and community committees for program planning.
  • Plans and conducts the Medical-Financial Partnership program under the guidance of Program Co-Directors.
  • Provides orientation to multi-disciplinary staff members to further their understanding of Medical-Financial Partnership program in mitigating impact of structural, economic, and social drivers of intergenerational health.
  • Provides linkage to community agencies to secure resources for clients, families, and significant others in order to achieve client's goals.
  • Serve as day-to-day manager of program clinical site operations.
  • Meets and/or huddles with the Medical-Financial Partnership team and staff daily.
  • Works with families and significant others toward gaining their acceptance of and participation in treatment recommendations.
  • Participates in multi-disciplinary team conferences to discuss and evaluate the nature of the client's goal(s), to evaluate the client's progress, and to develop a care plan.
  • May participate in research studies and special projects to develop more effective methods of identifying and evaluating the needs of patients and families.

REQUIREMENTS:
TRAINING AND EXPERIENCE:
A Master's Degree in Social Work from a graduate school accredited by the Council on Social Work Education including two years of clinical social work experience.
LICENSE:
  • A valid and active license as a Licensed Clinical Social Worker issued by the California Department of Consumer Affairs, Board of Behavioral Sciences.
  • Master of Social Work graduates must become registered as an Associate Clinical Social Worker with the California Department of Consumer Affairs, Board of Behavioral Sciences within sixty (60) days from commencement of employment with the County of Los Angeles. Active and valid Associate Clinical Social Worker registration must be maintained continuously until successful completion of the examination process for licensure as a Licensed Clinical Social Worker. Registered Associate Clinical Social Workers must obtain licensure within six (6) years from the original date of registration with the California Department of Consumer Affairs, Board of Behavioral Sciences. Failure to obtain licensure within the aforementioned timeframes may result in termination.
  • A valid California Class C Driver License or the ability to utilize an alternative method of transportation when needed to carry out job-related essential functions.

DESIRABLE QUALIFICATIONS:
• At least 5 years Post-degree experience working in a multidisciplinary team within a integrated care and/or medical setting assessing psychosocial needs of patients and families
• At least 5 years Post-degree experience providing clinical social work services in prenatal, infant, family, and /or early childhood mental health settings and developing interventions/care plans
• Able to work in a fast-paced environment with minimum supervision
• Spanish language fluency
