This full-time position offers a competitive annual salary ranging from USD 87,000 to 105,000, depending on experience and qualifications. This position is entirely remote, allowing for flexibility while providing care in an outpatient setting. • Conduct comprehensive patient intakes, focusing on diagnostic assessments and functional evaluations to inform treatment recommendations.
• Deliver targeted interventions for patients experiencing mental health challenges, providing brief, yet impactful therapy sessions tailored to individual needs.
• Collaborate with primary care providers to identify patients who require specialized mental health support and facilitate timely referrals when necessary.
• Develop personalized relapse prevention plans to aid patients in maintaining stability and managing their mental health proactively.
• Monitor patient progress through consistent follow-ups and adjustments to treatment plans, ensuring a continuity of care.
• Educate patients on self-management strategies and empower them with coping skills that enhance their engagement in treatment.
• Document all relevant patient information, progress notes, and treatment plans accurately in electronic health records.
• Participate in case consultations with clinic management and staff to improve service delivery and identify areas for evaluation and enhancement.

You will work with a diverse client population in a dynamic outpatient setting, focusing on individuals facing various mental health challenges. The counselor's role is integral to addressing psychosocial issues, thereby uplifting not only individual patients but also their families and the broader community.

• Master’s Degree in Clinical Counseling or a related field from an accredited institution.
• Current active Licensed Professional Counselor (LPC) license in Texas.
• Strong background in mental health assessment, treatment planning, and behavioral therapy methodologies.
• Thorough knowledge of DSM-IV diagnoses and the ability to apply diagnostic criteria appropriately.
• Ability to establish effective working relationships with patients, families, and healthcare professionals.
• Proficiency in navigating electronic health record (EHR) systems for documentation and patient management.

• Experience in a primary care or outpatient mental health setting.
• Familiarity with integrated care models and collaborative practices.
• Bi-lingual proficiency in English and Spanish is preferred but not required.
