Work Environment

This role is fully remote, allowing you the flexibility to engage with patients from the comfort of your own home while maintaining a strong connection with your colleagues through virtual collaboration. Your typical work hours will be Monday to Friday, 8:00 AM to 5:00 PM, with occasional adjustments to accommodate patient needs.

Key Responsibilities

• Conduct thorough and empathetic mental health assessments to gauge the psychological impact of patients' physical medical conditions.
• Develop and execute comprehensive treatment plans that consider both the psychiatric and medical facets of patients' care.
• Provide individual and group therapy sessions using evidence-based medication interventions tailored to meet each patient's specific needs.
• Collaborate actively with medical professionals to ensure seamless integration of care, ensuring effective treatment results for patients with complex medical histories.
• Offer crisis intervention services, supporting patients during mental health emergencies and providing necessary resources.
• Educate patients about the vital connection between mental health and physical well-being, empowering them to take an active role in managing their conditions.
• Maintain accurate, thorough, and confidential documentation to comply with legal, professional, and ethical standards.
• Participate in regular team meetings and case conferences, contributing to interdisciplinary care planning and continuous improvement of patient care.
• Stay informed about the latest research and best practices related to integrated health care, ensuring your therapeutic approaches remain current and effective.

Patient Population / Client Focus

You will be primarily focused on patients who are dealing with co-occurring conditions, such as those with chronic pain, autoimmune disorders, or substance use disorders. Our patients vary from young adults to seniors, each facing unique challenges that require a nuanced understanding of how mental health intersects with their overall physical health. Your role is crucial in devising treatment plans that consider not just psychological conditions, but also the broader health landscape affecting these patients.

Qualifications

• A Master's degree in Social Work, Mental Health Counseling, Marriage and Family Therapy, or Psychology from an accredited institution.
• A current and valid license as an LCSW, LCPC, or LMFT in the state of Illinois.
• Strong proficiency in addressing mental health needs specific to individuals with concurrent medical conditions.
• Exceptional interpersonal and communication skills that foster trust and rapport with patients and colleagues.
• Ability to navigate electronic health record (EHR) systems effectively as part of daily operations.
• Demonstrated capacity to maintain professional boundaries and comply with ethical guidelines and confidentiality standards.

Preferred Experience

• At least 2 years of experience working as a mental health therapist, preferably in an integrated care setting.
• Familiarity with best practices in interdisciplinary team care and collaboration with medical professionals.
• Experience in providing crisis intervention and managing mental health emergencies.

Compensation and Benefits
