Job Summary

The employee's number one job responsibility is to deliver optimal medication management across all dimensions of care every time. The pharmacy vision is to become world-class pharmacy recognized for excellence.
This individual will serve as a resource to the interdisciplinary team, and other pharmacists while delivering innovative practices and service delivery models that ensure cost effective and optimal medication management in the population we serve. Individual will participate in operational and clinical duties of a clinical pharmacist as assigned and required.
Multi-disciplinary rounds including clinical initiatives, direct patient care and educational/training activities will be performed. Individual will also provide education to technicians, pharmacists, and other health care practitioners. They will serve as a preceptor of pharmacy interns, technicians, and ASHP-accredited pharmacy residents.


Minimum Qualifications
Degrees:
  • Bachelor of Pharmacy Must have a Bachelor's Degree in Pharmacy or Pharm. D. degree from an accredited school of Pharmacy
  • Doctor of Pharmacy Preferred

Work Experience:
  • Three (3) Years Work Experience of progressive clinical practice within specialty area and completion of PGY1 from an ASHP-accredited residency program
  • (OR)
  • No Experience Required with completion of PGY1 and PGY2 from an ASHP-accredited residency program

Licenses & Certifications:
  • Adv. Cardiac Life SupportAdvanced Cardiac Life Support (ACLS) with course accredited by the American Heart Association (AHA) required for inpatient positions & Outpatient Oncology prior to or within 6 months of employment.
  • Preferred for All other Outpatient
  • IV Sterile Certification Required for all inpatient areas and Fondren Pharmacy Preferred All other Outpatient
  • Board Certified Pharmacy Specialist Board of Pharmacy Specialties Certification to be completed within 18 months, if not held at the time of appointment (unless previously grand-fathered)
  • Basic Life SupportAmerican Heart Association or Red Cross Basic Life Support (BLS) certification within 90 days of employment.
  • Neonatal Resuscitation ProgramOnly required for NICU/ EC Clinical Pharmacy Specialist
  • Pediatric Adv. Life SupportOnly required for NICU/ EC Clinical Pharmacy Specialist
  • PharmacistTexas Pharmacist License or eligible for Texas Pharmacist License; must obtain licensure within 90 days of employment.
  • Texas Pharmacist Preceptor Certificationwithin 90 days of employment
