Location: Lake Charles, LA
Pay Rate: $55.92/hour
Schedule: Monday–Friday, 8:00 AM–4:30 PM
Hours: 40 hours/week guaranteed
Position Type: Short-Term Contract
Coverage Area: Greater Lake Charles
Mileage: Reimbursed between patient visits
On-Call/Weekend: None
Dress Code: Navy blue scrubs preferred

Position Overview

We are seeking an experienced Travel RN – Hospice to provide compassionate, patient-centered care throughout the Greater Lake Charles area. The RN will be responsible for case managing a group of hospice patients, coordinating care with patients, families, physicians, and interdisciplinary team members while ensuring quality care and accurate documentation.

This is an excellent opportunity for a hospice RN who is comfortable working independently in the field and has a positive, flexible, and patient-focused approach.

Key Responsibilities
  • Manage an assigned caseload of hospice patients throughout the Greater Lake Charles area.

  • Conduct comprehensive nursing assessments and monitor patients for changes in condition.

  • Develop, implement, and update individualized hospice plans of care.

  • Provide skilled nursing care focused on comfort, symptom management, and quality of life.

  • Assess and manage pain and other end-of-life symptoms.

  • Administer medications and treatments as ordered and evaluate patient response.

  • Educate patients and families regarding medications, disease progression, symptom management, and hospice services.

  • Coordinate care with physicians, hospice aides, social workers, chaplains, and other members of the interdisciplinary team.

  • Communicate changes in patient condition and care needs to appropriate members of the care team.

  • Maintain timely, accurate, and complete clinical documentation.

  • Support patients and families through the hospice and end-of-life process with compassion and professionalism.

  • Travel between patient homes throughout the assigned coverage area.

  • Follow all applicable clinical, regulatory, and organizational standards.

Required Qualifications
  • Active Registered Nurse (RN) license.

  • Previous hospice nursing experience required.

  • Strong clinical assessment and patient-care skills.

  • Ability to independently manage a patient caseload in a home-based environment.

  • Excellent communication and organizational skills.

  • Valid driver's license and ability to travel between patient visits.

  • Patient-centered approach with a positive and flexible attitude.

Preferred Qualifications
  • Experience with Homecare Homebase (HCHB).

  • Previous hospice RN case management experience.

  • Home health or home-based nursing experience.

  • Experience coordinating care with interdisciplinary teams.

  • Strong knowledge of pain and symptom management and end-of-life care.

Pay Details: $55.92 per hour
