General Summary:
The Ambulatory Clinical Pharmacy Specialist provides ambulatory clinical pharmacy services. his role functions as a credentialed, privileged member of the healthcare team, practicing under collaborative practice agreements (CPAs) to optimize therapeutic outcomes, enhance medication access, and reduce the overall cost of care.
The Specialist collaborates with prescribers, nurses, and multidisciplinary team members to design, implement, monitor, and document therapeutic plans that reflect safe, effective, and evidence-based medication use. These activities are recognized nationally as Comprehensive Medication Management (CMM) and serve as the foundation of Bayhealth's ambulatory pharmacy model. The Specialist serves as a clinical resource for providers, staff, and patients while supporting CMM, chronic disease management, and specialty medication care coordination.
Responsibilities:
1. Provide ambulatory clinical pharmacy services under pharmacy CPA as credentialed and privileged by Bayhealth's Medical Executive Committee (MEC), including assessment, monitoring, and documentation of therapeutic plans.
2. Conduct patient visits for medication management, including, but not limited to, medication reconciliation, laboratory monitoring, patient counseling, and disease state management.
3. Collaborate with Bayhealth providers and care teams to implement evidence-based medication interventions that support quality, safety, and cost reduction goals.
4. Address referrals received from Bayhealth providers, ensuring closed-loop communication with referring clinicians.
5. Support patient access and affordability through coordination of prior authorizations, financial assistance programs, and benefit investigations.
6. Provide clinical oversight for patients on specialty medications, ensuring safe initiation, monitoring, and continuity of specialty medication therapy.
7. Provide drug information and education to patients, families, and healthcare team members.
8. Precept pharmacy students, interns, and/or residents as assigned.
9. Develop new and/or improve systems and/or services to enhance the level of care provided to patients.
10. Participate in research/publications related to applicable ambulatory and specialty pharmacy practice areas.
11. Participate in committee workgroups, policy development, and quality improvement initiatives that advance Bayhealth's ambulatory pharmacy services.
12. Document all interventions in the Bayhealth electronic health record and report outcomes consistent with Bayhealth and national standards for ambulatory pharmacy practice.
13. All other duties as assigned within the scope and range of job responsibilities.
Required Education, Credential(s) and Experience:
  • Education: Bachelor Degree Pharmacy
  • Credential(s): Licensed Pharmacist, Board of Pharmacy Specialties (BPS) Certified in a specialty; Board of Pharmacy Specialties (BPS) Certified in Pharmacotherapy, Ambulatory Care, Cardiology, or Geriatrics.
  • Experience: Required: One year of clinical pharmacy experience, including residency experience.
