Compensation: Pay per visit, $55 - $140 per visit (amount dependent upon visit type), $330 - $400+ per day, Mileage reimbursed at $0.67/mile, and Payroll processed bi-weekly (26 pay periods).
Shift Type: PRN (as needed), Minimum availability for 6 visits per week, Flexibility, In the field.

Summary:
We are looking for a compassionate Home Health Registered Nurse (RN) to work one-on-one with our patients in their home environment to give proper education and skilled nursing to allow our patients to become more independent. The Home Health Registered Nurse (RN) for this position enjoys working with a team of supportive clinicians as well as being disciplined to ensure the highest level of care.
Responsibilities:
โ€ข Assumes responsibility for all assigned patient care by effectively executing each patient's Plan of Care.
โ€ข Collaborates with the patient, family/caregiver(s), ordering physicians, other care team members, and community resources.
โ€ข Submit accurate, complete, timely, meaningful, and required documentation, including progress and clinical notes.
โ€ข Participate when possible in case conferences with other clinicians by reviewing services, clinical documentation, and discussing pertinent patient care issues and goals.
โ€ข Participate in on-call rotation as reasonably assigned.
Qualifications:
โ€ข Minimum of 2 years nursing experience as a Registered Nurse (RN) with a preference for 2+ years in an acute or a post-acute care setting.
โ€ข Active State Registered Nurse (RN) licensure is required.
โ€ข Proof of current CPR certification and valid Driver's license.
