  • Education: Minimum education is working towards an Associate’s Degree in Human Services or Bachelor’s Degree in Social Work
  1. Licensure: None required.

  • Clinical skills including in systemic family therapy, crisis intervention, family education, and linking/ advocacy skills.
  • Completion of Vroon Vandenburg High Fidelity Wraparound Training.
  • Core Expertise: Possesses skill, knowledge and abilities to perform the essential duties of their role; keeps knowledge up to date.
  • Independent Judgment: Ability to perform job responsibilities with a high degree of initiative and independent judgment.
  • Cultural Competency: Demonstrates awareness, sensitivity and skills in working professionally with diverse individuals, groups and communities who represent various cultural and personal background and characteristics.
  • Interpersonal Communication: Communicates clearly using verbal, nonverbal, and written skills in a professional context; demonstrates clear understanding and use of professional language.
  • Professional & Ethical Conduct: Adheres to professional values such as honesty, personal responsibility, and accountability; Applies ethical concepts within scope of work and adheres to Agency policies and procedures.

JOB SUMMARY:
The Care Coordinator Intern is available to students actively enrolled in Human Services or similar Associate’s or Bachelor’s programs. The Care Coordinator Intern, under the administrative/clinical supervision of the CME Clinical Supervisor, is responsible for learning about care coordination and providing care coordination services to clients and families identified by the CME Catchment Area (CCA). The Care Coordinator Intern helps coordinate formal and informal services for the child and family in accordance with the principals of the High-Fidelity Wraparound model associated with the CME project. This includes, but is not limited to, engaging families in Wraparound model, planning, implementing and monitoring service provision, and collaboration with involved partners.
ESSENTIAL DUTIES:                  
  1. Observe and learn intensive care coordination and case management services provided to children and young adults (up to age 21) with severe and persistent mental health needs.
  2. Shadow Care Coordinators in developing and maintaining service relationships with clients, families, and caregivers to understand engagement and support practices.
  3. Participate in structured service planning and care coordination activities through High-Fidelity Wraparound, including observation and supported involvement in care plan development and monitoring.
  4. Observe and assist Care Coordinators, under supervision, with coordination of referrals and linkage to appropriate community resources.
  5. Learn documentation standards and confidentiality requirements related to client and family interactions, including legal, financial, and clinical records, in accordance with agency guidelines.
  6. Participate in training related to cultural competency, trauma-informed care, and program requirements.
  7. Observe identification of urgent, unsafe, and crisis situations and appropriate escalation to supervisors.
  8. Attend and participate in agency, team, and clinical meetings, including mandatory trainings and treatment coordination activities.
  9. Observe and support discharge planning and transition activities as appropriate.
  10. Exhibit positivity, flexibility and a willingness to take on new responsibilities as requested or required.
  11. Demonstrate positive leadership, promote a team-based work environment and a culture of belonging.
  12. Present the Agency in the most positive light with all internal and external contacts.
