Job description

POSITION SUMMARY: The Clinical Quality Coordinator (CQC) plans, designs, implements and maintains a comprehensive practice of quality management in patient care and client services. The CQC collects and analyzes data, identifies quality improvement opportunities, leads quality initiatives and reports on efforts to improve health outcomes for patients/clients. The CQC develops strategies to meet objectives defined by agency leadership and program/contract requirements. The CQC works closely with clinicians, program managers, quality committee members and other assigned staff and serves as a liaison with Client Advisory Board (CAB) members and community partners.


ESSENTIAL DUTIES & RESPONSIBILITES:

  • Compile data and reports using AthenaOne, Dentrix, CAREWare and other systems.
  • Analyze and organize information in reports and presentation formats that support decision making, meet reporting and compliance requirements, drive performance improvement efforts and effectively communicate key information for various audiences.
  • Lead the multidisciplinary Quality Committee. Prepare agendas and activities; chair meetings; engage committee members in quality efforts; ensure that minutes and reports are created and disseminated in a timely manner.
  • Prepare and submit reports for executive leaders, program managers, internal teams, funders, partners and stakeholders.
  • Serve as the Quality Lead for HIV programs, cancer screening programs and other projects as assigned. Attend meetings with the Detroit Health Department, Michigan Department of Health & Human Services, CDC and other government and community partners.
  • Organize and implement Quality Plans, Quality Improvement Projects, Evidence Based Interventions, presentations and reports that meet program requirements.
  • Perform data entry for designated areas and monitor data completion and accuracy across programs.
  • Monitor performance measures and identify changes and trends. Prepare regular dashboards and reports. Work with leadership, program managers and internal teams to identify data and reporting needs. Create custom reports. Assist teams with data and report access and analysis.
  • Assess knowledge base and train staff in quality processes and interventions. Assume leadership or support roles in Quality Improvement Projects based on team capacity.
  • Assess staff education needs and work with leadership to prioritize training. Develop orientation and training programs for new and current staff, interns and community partners. Organize trainings and maintain records of staff education and competencies.
  • Coordinate quality and data preparations for site visits and audits. Support program managers with internal audit tools to assess data completion and accuracy.
  • Design and implement surveys, focus groups and other means of soliciting feedback from patients/clients, staff and community members/stakeholders. Collect, analyze and report feedback data. Support follow-up processes and responses.
  • Serve as staff liaison for the client advisory board. Represent the organization with external stakeholders, committees and coalitions.  
  • Interact with patients/clients to assess programs, gather feedback and promote quality efforts.
  • Other duties as assigned.


REQUIRED EDUCATION AND EXPERIENCE:

  • Master’s degree in public health, social work, nursing, or related field required. Extensive professional experience may be considered in lieu of degree requirements.
  • A minimum of 3 years’ experience in health care, public health or a related field including quality, data analysis and reporting responsibilities.


SUPERVISORY RESPONSIBILITIES: This position does not manage direct reports but may supervise interns.


TRAVEL: This position may require limited local and in-state travel primarily for meetings and occasional out of state travel for conferences/trainings.


SCHEDULE & WORKING CONDITIONS: This position is full time, predominantly during business hours Monday through Friday and may require some evening and weekend hours. The position is based in Detroit. Work is generally performed within the office environment and includes significant in person, phone and email contact with staff, patients/clients, funders and the general public. This position reports to the Chief Operating Officer.
