Case Manager
$25 - $27.50 / hour Flexible Schedule Work-Life Balance
Make a Difference with KDC
The Case Manager provides direct, in-home case management services for children and families within the Intensive Foster Care (IFC) and Lives-In Forward Transition (LIFT) programs. This role supports the development of safe, nurturing, and stable environments, collaborates with internal and external partners, and ensures compliance with all regulatory, licensing, and agency standards.
Schedule: Monday - Friday, 8 am - 4 pm
Location: New Bedford, MA
Key Responsibilities:
  • Assess the strengths, needs, and risk factors of children, youth, and families.
  • Build trusting, supportive relationships with children, youth, foster families, and biological families.
  • Provide in-home case management, supportive services, and advocacy.
  • Participate in treatment planning, IEPs, foster care reviews, and other team/case meetings.
  • Monitor progress toward treatment and educational goals and provide ongoing support.
  • Advocate for families and connect them with appropriate community resources and services.
  • Collaborate with internal teams, DCF, EEC, external agencies, and community providers to coordinate services and support placements.
  • Monitor placements to ensure compliance with DCF, EEC, and KDC requirements.
  • Complete timely and accurate documentation while maintaining compliance with agency policies and regulatory standards.
  • Provide transportation to appointments, programs, and placements as needed and perform other duties as assigned

Required Qualifications:
  • Bachelor's degree in Social Work or related field (Psychology, Human Services).
  • LSW/LSWA or obtained within 6 months of hire.
  • Completion and maintenance of CPR/First Aid certification.
  • Valid driver's license, acceptable driving history and reliable vehicle.
  • Proficiency in Microsoft Office.
