Job description

A community-focused, not-for-profit healthcare system southeast of Salem, Oregon is seeking an Advanced Practice Provider (Nurse Practitioner or Physician Associate) to join its Internal Medicine outpatient clinic in a collaborative, patient-centered environment.

Position Highlights:

  • Full-time outpatient Internal Medicine role (40 hours/week)
  • Adult primary care focused on chronic disease, acute care, and prevention
  • Manage a diverse, complex adult patient panel
  • Integrated multispecialty system (IM, FM, cardiology, surgery, pulmonology, ortho, women’s health)
  • Team-based care with physicians, MAs, behavioral health, care coordination, and CHWs
  • Strong emphasis on access, care coordination, and rural/community health
  • Participate in QI, virtual care expansion, and patient education initiatives
  • Supportive infrastructure built for efficiency, collaboration, and provider growth

Qualifications:

  • NP or PA licensed in Oregon
  • DEA license
  • BLS or ACLS certification
