Description:
The Medication History Pharmacy Technician is a certified pharmacy technician who supports the patient care team by obtaining and documenting an accurate list of medications taken at home and in an outpatient medical setting for patients being admitted to the hospital and patients receiving surgical care at IU Health Arnett Hospital. This role involves conducting thorough interviews with patients and their families, as well as contacting outpatient pharmacies, long-term care facilities, and other healthcare providers to verify medication information.
The technician enters the good faith effort medication list into the electronic medical record, ensuring accuracy and completeness in accordance with organizational standards and is available for pharmacists, providers, nurses, and other healthcare providers to support treatment planning, medication orders during hospitalization, and post-discharge care.
We desire technicians with 3-5 years of recent retail experience who have good organizational skills and strong multitasking skills. Candidates must have good oral and written communication skills, be comfortable with patient interactions and communicating with providers and pharmacists as well as telephone interview interactions with the patients we serve.
Additional Duties Include:
  • Maintaining professional communication with patients, families, and healthcare staff.
  • Updating discharge pharmacy contact information.
  • Pulling in outpatient vaccine administration information.
  • Adhering to all departmental workflows and standards.

Requirements:
  • High School Diploma/GED is required.
  • Requires Indiana Pharmacy Tech-In Training OR Indiana Pharmacy Technician License OR intern/extern license if enrolled in accredited college of pharmacy.
  • Requires Pharmacy Technician National Certification within one year of hire unless enrolled in accredited college of pharmacy or enrolled in or graduated from an ASHP/ACPE accredited Pharmacy Technician training program.
  • Requires the absence of criminal history related to the ability to work with drugs.
  • Requires strong mathematical calculation skills.
  • Requires ability to learn basic pharmacy technician skills in assigned areas and work under close supervision or under clearly defined procedures.
  • Requires ability to learn effective written and communication skills.
  • Requires ability to learn and execute customer services skills.
  • Requires basic proficiency typing and using Microsoft office applications.
