Hospice RN Case Manager

Founded as Boulder Hospice in 1976, TRU Community Care is a nonprofit healthcare organization serving Boulder, Denver, and surrounding counties. With a mission to affirm life at every step of the journey with illness and loss, TRU provides a continuum of care through TRU Hospice, TRU PACE (Program of All-Inclusive Care for the Elderly), TRU Palliative Care, TRU Grief Services, and Landmark Memory Care.

Title: Hospice RN Case Manager Pay: $88,067 to $105,680 per year Sign-On Bonus: $6,000 Location: Thornton and Westminster area; travel to patient homes and facilities required Schedule: Monday–Friday, 8am-5pm

Job Summary:

We are hiring a Hospice RN Case Manager to provide compassionate, patient-centered care to hospice patients and their families. This role supports patients in their homes, facilities, and community settings, helping manage symptoms, coordinate care, and support comfort and quality of life.

The RN Case Manager works closely with the hospice team, patients, families, caregivers, and providers to develop and carry out individualized plans of care. This is a hands-on, patient-facing role with regular local travel throughout the assigned service area.

Responsibilities:

  • Assess patient nursing needs, symptoms, pain, and changes in condition
  • Develop and update individualized plans of care in collaboration with the hospice physician, primary care provider, and interdisciplinary team
  • Provide skilled nursing care in patient homes, facilities, and community settings
  • Incorporate patient and family goals, preferences, cultural needs, and spiritual beliefs into the plan of care
  • Educate patients and families on medications, personal care, safety, nutrition, disease progression, and end-of-life care
  • Coordinate care with hospice team members, caregivers, providers, and community partners
  • Complete required documentation accurately and timely
  • Complete supervisory visits for CNA services at least every 14 days and as patient condition warrants

Qualifications:

  • Current RN license in the state of Colorado required
  • 2+ years of RN experience highly preferred
  • Prior hospice experience highly preferred
  • Valid driver's license, reliable transportation, and proof of auto insurance required
  • CPR certification required or ability to obtain within 60 days of hire
  • Ability to complete local travel to multiple patient care locations
  • Must comply with agency health screening and immunization requirements
