Location: Grand Junction, CO
Type: Contract | 12 Weeks (With a strong possibility of extension)
Shift & Schedule: Variable Days | 40 Hours Per Week

Pay Rate: $1,800/week to $2,200/week

Position Overview:

Navitas Healthcare, LLC is seeking a Hospice RN Case Manager for an exciting Travel or Local job in Grand Junction, CO.

Responsibilities

  • Manage and coordinate care for an assigned hospice patient caseload.

  • Conduct comprehensive patient assessments and develop individualized care plans.

  • Provide education and support to patients, families, and caregivers.

  • Collaborate with physicians and interdisciplinary healthcare teams.

  • Deliver expert symptom management and end-of-life care.

  • Monitor patient progress and adjust care plans as needed.

  • Maintain accurate and timely clinical documentation.

  • Provide care in patients' homes, assisted living facilities, nursing homes, and other community settings.

  • Participate in occasional on-call and weekend coverage as assigned.

Required Qualifications

  • Graduate of an accredited Registered Nursing Program.

  • Active Colorado RN License or Compact RN License.

  • Current BLS Certification.

  • Minimum 2 years of RN experience.

  • Strong assessment, critical thinking, and communication skills.

Preferred Qualifications

  • Previous Hospice, Home Health, Palliative Care, or Case Management experience.
