Job description

Founded as Boulder Hospice in 1976, TRU Community Care is a nonprofit healthcare organization serving Boulder, Denver, and surrounding counties. With a mission to affirm life at every step of the journey with illness and loss, TRU provides a continuum of care through TRU Hospice, TRU PACE (Program of All-Inclusive Care for the Elderly), TRU Palliative Care, TRU Grief Services, and Landmark Memory Care.
Title: Flex/Float RN
Schedule: Monday-Friday, 8am to 5pm
On Call Rotation: Current rotation is one weekend, done every 8 weeks.
Location: Varies throughout TRU's territory, based on assignment (Field-based North near Boulder/Longmont, field-based South near Denver, or Longmont Inpatient Unit)
Pay: $42.34 to $50.81 per hour
JOB SUMMARY
This position serves as a float RN, providing coverage for a variety of daytime operational needs across our hospice division. Depending on staffing and patient care needs, you may work as an RN Case Manager, Admissions RN, or Inpatient Unit RN.
One day, you may provide in-home hospice case management for patients in private residences and care facilities throughout the Boulder County area. The next, you may conduct hospice admission visits in the Parker and Denver regions. On another day, you may support patient care in our inpatient hospice unit located at Longmont United Hospital.
This role offers a dynamic and rewarding opportunity for an adaptable nurse who enjoys variety, values teamwork, and is committed to delivering compassionate, high-quality hospice care across multiple care settings.
ESSENTIAL DUTIES AND RESPONSIBILITIES

  • Comprehensively assesses and anticipates all aspects of the patient's clinical nursing needs, pain and other symptoms and, in collaboration with primary care physician or hospice physician, develops an individualized plan of care.
  • Provides unsupervised CNA visit at least every 14 days and as patient condition warrants.
  • Incorporates patient/family preferences and goals within the plan of care.
  • Supports and respects the patient's and family's unique diversity, including spiritual and cultural beliefs.
  • Provides patient and family education as appropriate; areas of education include, but are not limited to personal care, medications, procedures, safety, diet and nutrition, disease process, death and dying.
EDUCATION & REQUIREMENTS
  • Must possess a current RN license in the state of Colorado.
  • 2+ years as an RN, highly preferred
  • Prior experience working in hospice, highly preferred
  • Must comply with agency health screening and immunization requirements.
  • Must successfully pass required background checks and drug screening
  • Requires travel to multiple work locations
