Reports To: Director of Aging Services

Compensation
  • BS (or equivalent): $50 โ€“ $70

Role Summary

As Aging Care Manager, you assess, plan, coordinate, and monitor care services for elderly clients. You work closely with clients, their families, and healthcare providers to ensure highโ€‘quality support.

Responsibilities
  • Client assessment and care planning
    • Conduct comprehensive assessments of clientsโ€™ physical, emotional, social, and environmental needs.
    • Develop individualized care plans based on assessment findings, client goals, and family input.
    • Monitor and revise care plans as needed.
  • Care coordination
    • Facilitate communication between clients, families, healthcare providers, and other support services.
    • Arrange medical, social, and personal care services, including home health care, transportation, and meal services.
    • Advocate for clientsโ€™ needs and preferences in all aspects of their care.
  • Resource management
    • Identify and connect clients with community resources and support services.
    • Assist families in navigating the healthcare system and accessing benefits.
    • Provide education and support on agingโ€‘related issues.
  • Monitoring and followโ€‘up
    • Conduct regular followโ€‘up visits and checkโ€‘ins to monitor wellโ€‘being and care plan effectiveness.
    • Adjust care plans as necessary.
    • Document all client interactions, assessments, and updates.
  • Family support
    • Offer counseling and support to families.
    • Provide guidance on longโ€‘term care planning, legal issues, and financial considerations.
    • Facilitate family meetings and decisionโ€‘making processes.
Qualifications
  • Masterโ€™s degree in Social Work, Nursing, Gerontology, Occupational Therapy, or related field.
  • Professional licensure in good standing (RN, NP, LMSW, or equivalent).
  • Minimum 5 years of direct care experience in care management, home care, acute care, or rehabilitation settings.
  • Strong knowledge of aging issues, healthcare systems, and community resources.
  • Experience working closely with families and professional providers.
  • Ability to work independently and as part of a multidisciplinary team.
  • Excellent written and oral communication, interpersonal, and organizational skills.
  • Comfortable and quick to learn technology (e.g., Microsoft Office, Salesforce).
  • Valid driverโ€™s license and reliable transportation.

  • Experience in the behavioral health field.
