Job description

PHYSICAL THERAPIST CONSULTANT

Mozaic (A Chapter of The Arc NY) is seeking a dedicated and compassionate Physical Therapist Consultant to join our team in Ontario County. In this role, you will provide high-quality therapeutic services that enhance independence, improve functioning, and reduce the impact of disabling conditions for the people with intellectual and developmental disabilities.

You will work collaboratively across teams to help people we support remain in, or transition to, the least restrictive environment possible, ensuring they have access to the support they need to succeed in their daily lives. This is a highly specialized position requiring strong clinical expertise, effective communication skills, and a commitment to person-centered care.

Pay Range: $50-$90/hour - Fee for service contract

Location: Ontario County

Schedule: 6-8 Hours per week

8am - 5pm with flexibility

Physical Therapist Duties

You will design and deliver individualized physical therapy programs that promote mobility, independence, and overall quality of life. Using your clinical expertise, you'll assess physical functioning and develop personalized treatment plans that help the people we support build strength, coordination, and essential daily living skills.

In addition to providing hands-on therapy services, you will incorporate adaptive equipment and therapeutic strategies to maximize independence and prevent further disability. You will collaborate closely with families, residential teams, day programs, and community providers to ensure coordinated, person-centered care.

You will also serve as an advocate for the people we support, contributing to interdisciplinary planning and supporting staff through training and consultation to ensure therapy programs are implemented effectively.

This dynamic role offers variety, including working across different settings and adapting your schedule as needed to meet service needs, all while building meaningful connections and helping others live more independently and fully.

Minimum Qualifications:

Licensed and currently registered as a Physical Therapist by the NYS Education Department. Experience with the developmentally disabled community preferred.
