Licensed Master Social Services Assistant

Job Description

PRIMARY DUTY

The primary purpose of the Social Service Assistant is to assist the Social Service Director in providing services to meet the social and/or emotional needs that affect the residents ability to achieve their highest level of function; participate in the development of residents comprehensive care plans; develop policies and procedures to provide social services to residents in compliance with federal, state and local regulations.

QUALIFICATIONS/SKILLS

Licensed Master Social Worker Required

Minimum of 1 year of supervised social work experience in health care setting working directly with individuals preferred.

Excellent organizational skills and the ability to multi-task.

Possess ability to communicate well with families and residents.

Must possess the ability to make independent decisions when circumstance warrants such actions.

Must possess the ability to deal tactfully with personnel, residents, family members, visitors, government agencies/personnel and the general public.

Must possess leadership ability and willingness to work harmoniously with all personnel.

Proficient in Microsoft Excel.

EXPECTATIONS OF EMPLOYEE

Observe all facility safety policies and procedures.

Complies with state, federal, and all other applicable health care and safety standards

Follow facility Residents Rights policies.

Protect the confidentiality of all resident information.

Assists families and other visitors as needed.

Attends/completes required in-services and other required meetings.

Come to work as scheduled and consistently demonstrate dependability and punctuality.

Assume accountability for data contained in the employees handbook.

Perform all duties assigned in an effective, timely and professional manner.

Accept assigned duties in a cooperative manner.

Perform other related duties as directed by his/her supervisor.

ESSENTIAL JOB FUNCTIONS include the following. Other duties may be assigned. Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions

Develop, coordinate and participate in family and resident activities designed to promote social interaction, reality orientation and intellectual stimulation.

Develop one-to-one professional relationships with residents and families as needed for counseling.

Counsel residents and families in dealing with feelings about death or dying and other emotional, mental environmental or physical limitations.

Assess and complete the sections of the (MDS) Minimum Data Set assigned completely, timely and accurately.

Document psycho-social needs, interaction and follow-up action(s) with resident and/or family according to facility policies and procedures.

Assess, plan and document residents discharge needs in accordance with facility discharge planning policy and regulatory requirements.

Document the social service component of the Comprehensive Care Plan for each resident in a timely manner.

Refer residents to social, health and community agencies and complete accurate documentation in residents records concerning the results of such referrals.

Coordinate family and community resources as needed to solve financial needs and to promote emotional security.

Organize community events related to residents needs and interests.

Explain residents rights to residents and residents representatives.

Participate in admission of residents as assigned.

Plan, schedule and implement room visits for residents unable to leave their rooms.

Be responsible for the safety of residents under his/her supervision.

EDUCATIONS and EXPERIENCE an equivalent combination of education, training and experience will be considered.

SUPERVISORY REQUIREMENTS of this position are generally as follows:
