Job description

The Non-Clinical Case Manager coordinates referrals, service planning, and client documentation for designated caseload(s).

Minimum Requirements:

  • Professional License in Behavioral Sciences, Human Services, or Social Services preferred
  • Bachelor’s Degree in Behavioral Sciences, Human Services, or Social Services fields preferred
  • Experience in Case Management, Behavioral Sciences, Human Services or Social Services preferred
  • Current CPR if applicable
  • TB questionnaire, PPD or chest x-ray if applicable 
  • Current Health certificate (per contract or state regulation) 
  • Must meet all federal, state and local requirements 
  • Must be at least 18 years of age  
Facility Job Description:
  • Master's Required
  • Local Pay: $50-$55/hr
  • M-F 8a-430p
  • 37.5-40 hrs/week
  • 6 Month Contract
  • Start: Any Monday active
The focus of the Intensive Needs Service Coordinator is to coordinate treatment and supervision of individuals with developmental disabilities who pose a risk to the community. The Intensive Needs Service Coordinator assists the individual receiving services in accessing supports they are funded for through the State Developmental Disabilities Services Division (DDSD). The Intensive Needs Service Coordinator provides coordination to adults who have developmental disabilities whose waivers are primarily funded through the public safety priority of who pose a moderate to significant risk to themselves or others. All services are provided in a home and community-based setting while following Medicaid, State, and agency guidelines. The Intensive Needs Service Coordinator works closely with the individual receiving services, the case manager that we partner with through the Case Management Organization, the paid service providers and the guardian when applicable. Together the team works to support the individual receiving services in making gains toward meeting their goals.
