Registered Nurse (RN) – Home Health Visits | Metro Detroit

Position Overview
We are seeking compassionate and skilled Registered Nurses (RNs) to join our home health team in the Metro Detroit area. This is a contingent, visit-based role offering flexibility and autonomy while delivering high-quality, one-on-one care to patients in their homes.

The majority of visits will focus on wound care, including patients with wound vacs, along with tracheostomy (trach) care. Additional responsibilities include Start of Care (SOC), supervisory visits, and skilled nursing visits. This position is ideal for nurses who value independence, meaningful patient relationships, and a flexible schedule.

Key Responsibilities

  • Provide skilled nursing care in accordance with physician orders and care plans
  • Perform advanced wound care, including wound vac management
  • Provide and manage tracheostomy (trach) care
  • Complete Start of Care (SOC) and supervisory visits
  • Accurately document patient care using electronic medical records
  • Educate patients and families on care plans and health management
  • Collaborate with interdisciplinary care teams to ensure optimal patient outcomes

Qualifications

  • Active Registered Nurse (RN) license in the State of Michigan (Required)
  • Valid driver’s license and reliable personal transportation (Required)
  • Ability to work independently in a home setting
  • Comfortable using EMR systems via tablet or smartphone
  • Must pass a criminal background check
  • Must be authorized to work in the United States

Preferred Qualifications

  • Experience with wound vacs and tracheostomy (trach) care
  • Home health experience
  • COVID-19 vaccination
