This is a fee-for-service position. In order to be considered for this job you MUST have a valid RN license in the state of NY and prior UAS experience.

Rates are paid per visit.

Pay: Initial visit- $165.00 per visit; Reassessment- $140.00 per visit

JOB SUMMARY

We are seeking a dedicated and compassionate Nurse to join our healthcare team. This role is essential in providing high-quality patient care in a dynamic environment, whether in a teaching hospital or urgent care setting. The ideal candidate will possess strong clinical skills and a commitment to patient education and advocacy. As a UAS Assessor, you will conduct in-person assessments for clients to evaluate their eligibility and ongoing needs, contributing to crucial care and support plans.

RESPONSIBILITIES:

· Conduct UAS assessments for clients to determine their level of care and support needs.

· Utilize the Uniform Assessment System (UAS) tool to document assessment results accurately.

· Provide compassionate and professional interactions with clients in various community settings.

· Submit completed assessments within designated timeframes and maintain detailed records.

· Collaborate with other care providers to ensure a comprehensive assessment process.

· Attend required training sessions and stay informed of assessment updates.

QUALIFICATIONS

  • Registered Nurse, currently licensed and registered to practice in New York State.

  • UAS-NY Certified

  • Bachelor’s degree in nursing preferred.

  • Experience in Home Care, case management, or long-term home care.

  • Demonstrated skills in planning, organizing and directing the work of others required.

  • Experience in Intake or an admission team of PACE is highly desirable, including completion of UAS-NY Assessments.

  • Proficient with DOH UAS assessment instrument.

Experience:

  • UAS-NY Certified

License/ Certification:

  • BLS Certification (Preferred)

  • RN License (Required)
