Description

• Conduct assessments in inpatient or outpatient settings and formulate personalized treatment plans
• Implement physical therapy interventions aimed at enhancing mobility, strength, balance, and overall function
• Work collaboratively with nurses, physicians, case managers, and other rehabilitation specialists to ensure exceptional patient outcomes
• Inform patients and their families about therapeutic objectives, discharge plans, and home exercise regimens
• Keep comprehensive records of evaluations, treatments, and patient progress in line with facility and regulatory standards
• Engage in care team discussions and interdisciplinary meetings to foster coordinated care practices

Education

• Doctorate in Physical Therapy (DPT) or an equivalent degree from a recognized institution

Certifications

• Valid state licensure as a Physical Therapist
• CPR certification encouraged
• Additional certifications (e.g., neurological, geriatric) are advantageous but not required
