Activa Home Health, a licensed, Medicare-certified agency, is seeking skilled and compassionate Registered Nurses to provide Medicare home health visits in both full-time and per-diem roles. This is an exciting opportunity, and we are looking for Nurses across multiple communities in Indian River, St. Lucie, and Martin Counties, delivering high-quality, patient-centered care in the comfort of patients' homes.

Key Responsibilities
  • Deliver skilled nursing care per the Plan of Care during Medicare home health visits
  • Accurately document visits in the EMR, including OASIS assessments
  • Communicate changes in patient condition promptly with the interdisciplinary care team
  • Provide clinical services within scope of practice, including:
    • Medication administration
    • Wound care
    • Infusion therapy
    • Catheter care
    • Safe transferring and positioning of patients
  • Collaborate with Nurse Supervisors and Managers of Clinical Practice
  • Demonstrate professionalism, empathy, attention to detail, and strong clinical judgment

Required Qualifications
  • Active Florida Registered Nurse (RN) license
  • Medicare home health experience required
  • OASIS experience required
  • Wound care experience required
  • Proficiency with EMR systems (WellSky Kinnser preferred)
  • Minimum 1 year of home health experience
  • Strong communication, teamwork, and patient-focused skills
