  • Ensures continuity of care for patients managing complex therapies outside traditional clinical settings

Role Details

Location:
LUX Infusion Pharmacy is seeking a Per Diem Infusion Nurse to see patients in their homes in Albany, NY.
Work location is subject to change based on business needs.

On-Call Availability: Candidate is available to participate in the required on-call rotation, including nights and weekends. Understands that the role requires on-call coverage approximately once every eight weeks, consisting of two weekdays per month and one weekend rotation.

What You'll Do

Responsibilities include:

  • Provide direct home infusion nursing care by inserting, monitoring, and removing IVs and administering prescribed medications, blood products, and nutrition
  • Monitor patients for adverse reactions, obtain vital signs, complete physical assessments and reviews of systems, and document all patientrelated activities
  • Perform venipuncture and manage venous access, including peripheral IVs and central lines
  • Teach patients and caregivers how to safely perform and monitor home infusion therapy
  • Collaborate closely with company directors, nursing coordinators, directors of nursing, physicians, agencies, and internal teams
  • Coordinate startofcare and ongoing nursing visits and confirm schedules with patients and nursing partners
  • Complete, review, and submit nursing notes, therapy administration reports, care plans, and progress documentation
  • Communicate patient status changes, concerns, or reportable events to referring physicians and internal stakeholders
  • Maintain accurate documentation in accordance with clinical, regulatory, and company standards
  • Support staffing coordination while maintaining confidentiality and ethical standards
  • Drive within the state to patient residences and assigned care locations
  • Uphold compliance with all applicable federal and state laws and organizational policies

What We're Looking For

You may be a great fit if you bring:

  • Deep care about patient outcomes and homebased care experiences
  • A nursing degree from an accredited college or university
  • A current RN license in New York and at least two years of RN experience
  • IV certification and experience with central line care and management
  • Strong medicalsurgical, phlebotomy, and venous access knowledge
  • Confidence working independently while coordinating with multidisciplinary teams
  • Excellent assessment, documentation, organizational, and multitasking skills
  • Clear, compassionate communication with patients, caregivers, and providers
  • Comfort managing care in highstress or urgent homebased situations
  • Willingness to travel to patient homes and healthcare facilities

Preferred (but not required):

  • Bachelor's degree
  • RN licensure in additional states
  • Prior experience in home infusion therapy
  • PICC, port, or midline experience and certification
