Description:

Wondercare, Inc., a Partner of Soleo Health, is seeking a Per Diem Home Infusion Nurse to support our Bronx, NY and surrounding area patients.


** Must Have Home Infusion experience **


** Must live within 25 miles of the Bronx, NY area**


The Position:

The Infusion Nurse provides primary patient care for specifically assigned patients to include assessments, care planning, evaluation and education of patients receiving infusion therapy and/or other nursing services in accordance with the care matrices and best practices in the home or alternate site settings. Responsibilities Include:


  • Provide the majority, if not all of the direct care for all specifically assigned patients including initiation and maintenance of all communications with appropriate care team members including pharmacists, physicians, dieticians, home patient representatives and any appropriate community resource staff members.
  • Evaluate and educate patient/care partner’s ability to safely administer the home infusion therapy and provide additional instruction as indicated.
  • Evaluate the home environment where the care is to be provided and identify interventions that may be required to render the home care environment safe and appropriate for the prescribed care.
  • Develop, in conjunction with other clinical team members, each patient’s Care Plan and evaluate patient’s response to therapy during each home visit or encounter.
  • Appropriately discharge primary patients and complete discharge summary within 7 days of discharge.
  • Develop and maintain caseload scheduling and communications on a weekly and monthly basis in updating schedule with changes.
  • Document patient visit to physician or lab drawing station on calendar.
  • Coordinate care via live telephone conversations and/or voicemail with the team as determined by the urgency of information to be relayed.
  • Document all communications at the time of service.
  • Rotational after hour and weekend on call, and as needed
  • Computerized documentation of patient visits accurately and in a timely manner

Schedule:

  • Must be able to work Per Diem up to 20 hours per week, variable schedule
  • Must have AIS or Home Infusion Nursing experience
  • Must be knowledgeable of IV access devices to include: IV Lines, ports, and administration of infusion medications
  • Must be able to provide infusions to patients in their homes
  • Must be able to drive up to 45 miles to see patients as needed (we offer door to door hourly rate of pay!)
  • Must have properly registered/insured vehicle to see patients
  • Must be licensed as an RN in the state of New York
Requirements:
  • Minimum 3 years nursing experience with background infusion therapy
  • RN degree- BSN preferred
  • Current nursing license for the state of New York in good standing
  • Must have AIS or Home Infusion RN Nursing experience
  • Experience in ICU, or Cardiac Care preferred
  • Excellent customer service skills
  • Proficient in Microsoft Excel, Word, and Outlook (preferred experience using: CPR+, Insight, AlayaCare, and Concur)
  • Knowledgeable and competent in care and maintenance of vascular access devices
  • Competent in peripheral IV placement skills and port accessing (powerports, mediports, infusaports, portacaths)
  • Current CPR/BLS Certification
  • PICC/Midline placement a plus
  • CRNI a plus
  • Must own properly registered and insured vehicle to use for patient visits
  • Valid state driver’s license
  • Ability to multi-task and have critical thinking skills
  • Road warrior
