Job Title: Community Support Individual (Bilingual)

Job Code: HCP100/Behavioral Health Counselor 1

Shift: Full-Time (40 hours per week on average)

Division/Department/Program: Alianza Therapeutica Latina (ATL)

View Point Health proudly provides outpatient treatment to children, adolescents and adults. At the outpatient centers, we promote overall health and quality of life for individuals, youth and families through evidenced-based therapies and medication management to reduce the impact of mental illness, behavior challenges, and substance use.

Under general supervision, counsel with emphasis on prevention and/or behavior modification. Works with individuals and groups to promote optimum mental health. May help individuals deal with addictions and substance abuse, family, parenting, and marital problems, suicide, stress management, problems with self-esteem, and issues associated with aging and mental and emotional health.

Duties & Responsibilities:

  • Assist clients in accessing community resources, including housing, food stamps, Medicaid, vocational assistance, and transportation.
  • Provide rehabilitation services to develop coping skills, symptom management, and independent living skills (e.g., budgeting, housekeeping, hygiene).
  • Collaborate with therapists, nurses, and case managers to develop and implement Individualized Service Plans (ISP).
  • Conduct home or community visits to monitor client progress, encourage treatment compliance, and ensure safety.
  • Actively participate in crisis prevention and stabilization; assist in developing crisis plans and, when necessary, help connect clients to higher levels of care.
  • Maintain accurate, timely, and clinical-grade records of all client encounters, including progress notes, within the Electronic Health Record (EHR) system, within 24 hours.
  • Act as an advocate for the consumer in various settings, including healthcare or housing, to ensure rights are respected
  • Maintain accurate mileage logs and sign in sheets.
  • Participate in parents centered, school and community events.
  • Maintain updated community resources for school and clients.
  • Conduct/participate in mental health presentations at schools and community events with school counselors to update case notes.
  • Assesses and addresses school related issues: behavior, attendance, absenteeism, and barriers.
  • Establishes and maintains relationships with other community providers in outreach services, school activities, community fairs.
  • Links caregivers with community resources according to their needs (financial, job, training, Medicaid, food stamps)
  • Conducts three college, training, or careers activities for client members per year.
  • Provides strength-based interventions and developing skills that are directly associated to the client's individual target behavior and related back to their treatment plan.
  • Completes CANS assessments quarterly.
  • Provides and bills for services; Billing targets are required.
  • Maintain update authorizations for billing and notify counselors when authorizations are needed.
  • Data program collection (academic, assistance, behavior, surveys, etc.) and provide information to support staff.
  • Participate in weekly and monthly meetings.
  • Other related duties as assigned.

Minimum Qualifications:

Bachelor's degree in counseling or a related field from an accredited college or university or certification as an addiction counselor from the Georgia Addiction Counselors Association (CACA) or equivalent certification.

Note: Position requires ability to read, write, and speak fluently in English and Spanish.

Preferred Qualifications:

  • Experience with Care Logic or other EMR.
  • Comfortable and proficient with collaborative & concurrent documentation.
  • Experience in trauma-informed care, or trauma related interventions and skills.
  • Experience working with youth/children.
  • Excellent written and verbal communication skills.
  • Ability to be flexible and work in rapidly changing environment-DBHDD.
  • Must pass criminal records fingerprint check.

Requirements/Competencies:

  • Must be able to lift 20 pounds.
  • Requires long periods of sitting/standing.
  • Must have valid Georgia driver's license and Motor Vehicle Record in accordance with company policy.
  • Candidates for selection must pass a criminal background check (including fingerprinting).
  • Must pass a pre-employment drug screen and subject to random drug screens.
  • Fully Bilingual and Must be able to write, read, speak and comprehend English and Spanish.

Note: Some positions may require a valid driver's license.
