The Growth Intern supports outreach efforts across the full patient acquisition pipeline, from initial lead engagement through claim filing. This role assists Outreach Coordinators by coordinating patient outreach, scheduling and confirming appointments, screening for eligibility and clinical urgency, tracking required milestones, and maintaining accurate Salesforce documentation. Success is measured by supporting timely patient progression through each stage of the pipeline, including completed intakes, testing completion, claims filed, and accurate documentation that provides visibility into performance and operational outcomes.
This is a temporary contract position averaging 35 hours per week through December 2026, with potential for full time conversion based upon business needs.
Qualifications
  • High school diploma or equivalent required; Associate's or Bachelor's degree preferred.
  • Experience in high-volume customer service, call center operations, healthcare intake, or sales preferred.
  • Strong Salesforce or CRM documentation skills.
  • Exceptional verbal and written communication skills.
  • Strong organization, follow-through, and ability to meet KPI expectations in a fast-paced environment.
  • Patient-first approach consistent with HEART values.

Responsibilities
Lead Engagement and Scheduling
  • Contact all new leads.
  • Schedule intake appointments and ensure the Intake Appointment Date/Time is entered accurately in Salesforce.
  • Confirm intake appointments with patients the day prior.
  • Follow up when initial contact attempts are unsuccessful.
  • Process referrals from all sources and ensure documentation is uploaded before or after intake as appropriate.

Patient Qualification
  • Evaluate patients based on program criteria and level of need.
  • Qualify leads.
  • Assess and document patient acuity and urgency.
  • Communicate qualification findings to Outreach Coordinators for prioritization.

Salesforce Data Management and Ownership
  • Document all patient interactions, intake schedules, triage information, demographics, claim information, and updates in Salesforce.
  • Ensure scheduled and completed intakes, testing completion, and claims filed are trackable in Salesforce.
  • Maintain accurate Lead Owner and Visit information so reporting reflects individual and team performance.

Intake Conversion Support
  • Support Outreach Coordinators in converting leads to scheduled intakes and eventual home health care patients.

Team Collaboration
  • Partner with Sales and Marketing leadership to optimize the intake pipeline.
  • Participate in team huddles and strategy meetings to review KPI progress and barriers.
