WCS Healthcare is seeking an experienced Registered Nurse (RN) with UAS Assessment experience to join a respected Home Health organization serving Dutchess County, NY.

This is a fantastic opportunity for a nurse who enjoys working independently, building meaningful relationships with patients, and maintaining a healthy work-life balance through a flexible hybrid schedule.

What You'll Do

As a UAS Assessment Nurse, you'll play a key role in evaluating patients and connecting them with the appropriate home care and long-term care services.

Responsibilities:
  • Conduct UAS-NY assessments in patients' homes throughout Dutchess County
  • Evaluate physical, cognitive, psychosocial, and functional needs
  • Complete accurate and timely assessment documentation
  • Determine eligibility for home care and long-term care programs
  • Develop care recommendations based on assessment findings
  • Collaborate with physicians, care managers, caregivers, and interdisciplinary teams
  • Educate patients and families on available services and community resources
  • Maintain compliance with agency policies and New York State regulations
QualificationsRequired:

Active New York State Registered Nurse (RN) License

Current UAS-NY Certification

Previous UAS Assessment experience

Strong assessment, documentation, and organizational skills

Ability to work independently and manage a field-based schedule

Preferred:
  • Home Care experience
  • MLTC experience
  • Case Management experience
  • Community Health experience
Schedule
  • Full-Time
  • Monday-Friday
  • 9:00 AM-5:00 PM
  • 3 days conducting assessments throughout Dutchess County
  • 2 days working remotely from home
