Registered Nurse Case Manager

Location: Lebanon, New Hampshire

Pay Rate: $2,400 to $2,520/week

Contract: 13 weeks

Schedule: Day Shift

Guaranteed Hours: 36 hours per week

Experience: Minimum 2 years required

About the Role

We are seeking an experienced Home Health Registered Nurse Case Manager for a 13-week contract opportunity serving Vermont and New Hampshire communities. This role supports home health patients through assessment, care planning, case management, patient education, and coordination with physicians, caregivers, and interdisciplinary care teams.

Responsibilities

• Screen, evaluate, re-evaluate, and treat home health patients

• Develop and update individualized care plans with physicians, patients, families, and caregivers

• Coordinate patient care through interdisciplinary communication and case conferences

• Assess clinical, functional, and emotional status and report significant changes

• Document evaluations, treatment goals, care updates, and patient education accurately using EPIC

Requirements

• Active Vermont And New Hampshire Registered Nurse License Or Compact RN License Required

• Graduate of an accredited school of nursing required

• Minimum 2 years of Home Health RN experience required

• BLS/BCLS Certification Required

• Strong case management, care planning, patient education, documentation, and interdisciplinary communication skills required
