Signature Albany Home Health is seeking a skilled and compassionate RN Case Manager / SOC RN to provide high-quality home health care to patients in the comfort of their homes.

This full-time role is ideal for an RN with experience in home health, Start of Care (SOC) assessments, care coordination, and patient-centered care, who thrives in an independent, field-based environment while collaborating with an interdisciplinary team.

What You'll Do

Patient Care & Case Management

  • Perform comprehensive nursing assessments, including SOC visits, and develop individualized plans of care.
  • Coordinate and manage home health services in collaboration with physicians and care teams.
  • Provide skilled nursing care, treatments, and patient education.
  • Monitor patient progress and communicate changes in condition to physicians.
  • Implement preventive, rehabilitative, and restorative nursing interventions.
  • Identify and coordinate discharge planning needs.

Care Coordination & Documentation

  • Maintain accurate and timely clinical documentation and patient records.
  • Communicate with healthcare providers and community resources.
  • Ensure access to necessary medical equipment and services.

Leadership & Supervision

  • Supervise and evaluate Home Health Aides (HHAs), CNAs, and LPNs/LVNs.
  • Provide guidance to ensure quality care delivery and compliance.

Additional Responsibilities

  • Participate in quality assurance and performance improvement programs.
  • Attend training and in-service education.
  • Participate in on-call rotation as required.
What You'll Need
  • Graduate of an accredited nursing program.
  • Active Registered Nurse (RN) license in Oregon.
  • Minimum 2 years of nursing experience, preferably in home health or public health.
  • Experience completing Start of Care (SOC) assessments preferred.
  • Current CPR certification.
  • Valid driver's license and reliable, insured vehicle.
