Minimum Requirements

  • Bachelor's, Master's, or Doctoral Degree in Physical Therapy from an accredited program approved by the APTA.
  • Active Texas Physical Therapy license required to start the assignment.
  • BLS Certification from AHA or ARC may be required.
  • Home health experience required. Must have experience with home health evaluations, start-of-care visits, 30-day reassessments, treatment visits, and discipline discharges.
  • Familiarity with HCHB electronic medical record preferred. Candidates with HCHB and home health experience will be prioritized.
  • Ability to work as an independent contractor and document per agency standards.
  • Minimum education requirement: Bachelor’s Degree.

Assignment Details

  • Facility type: Home Health.
  • Shift name: Days.
  • Shift schedule: Days 08:00–16:00 (client also lists 08:00–16:30 consistently as expected daily schedule).
  • Weekly hours guarantee: 36 hours per week.
  • Coverage areas include Crockett, Grapeland, Elkhart, and Palestine. Mileage reimbursed between patient visits from first to last patient.
