Job description

Full-time, 35 hours per week
Hybrid up to 3 days per week once training is complete.
Responsible for functioning as a case manager within the Primary Care Team (PCT) for members participating in a Senior Care Option (SCO) plan. Make recommendations, coordinate, implement, and monitor contracted and community services for the purpose of improving or maintaining the member's overall health and functional status. Conduct periodic reassessments to determine appropriateness of the service plan and ensure that members have access to public benefits to support their wellness, independence, with the goal of preventing premature institutionalization.
Travel reimbursement is $0.70/mile
Minimum Qualifications: BA or BS in Social Work, Human Services or related field, experience in case management with geriatric population. LSW a plus. Must have valid driver's license and/or reliable transportation. Spanish/English fluency required. Must be able to climb multiple flights of stairs.
