LinkJobs works with large health care systems, small clinics and everything in between, and we are currently searching for the right person for the following position:
Home Visiting Nurse Practitioner (1099 Contractor)
Responsibilities: Manage the transitional and chronic care needs of homebound patients including:
* Assessment and evaluation of current condition* Obtain past medical history
* Review symptoms
* Conduct general physical examinations
* Review and reconcile medications
* Screen for depression
* Screen for alcohol and drug abuse
* Identify diagnoses to support care management and treatment planning
* Recommend appropriate measures, including medication prescriptions and skilled care needs
* Efficiently communicate assessment findings to the office/supervising physician
* Educate patients on disease processes, prevention, medications, and compliance
Qualifications:
* Current Nurse Practitioner license in the state of Georgia with no sanctions or restrictions* Minimum of 1 year of clinical experience
* Clinical background in Adult, Family, or Geriatric specialties preferred
* Experience using Electronic Health Records (EHR)
* Strong communication skills
* Comfortable using smartphone and email
* Reliable transportation required
* Ability to work independently
* Home care or home visiting experience preferred
* Must maintain active individual malpractice insurance coverage
* Willingness to spend 30 minutes to 1.5 hours with patients in their homes, which may include pets and/or household tobacco use
