Contracted In-Home Occupational Therapist

We partner with health plans to make homes safer and more accessible for older adults and high-needs populations. We achieve this by combining technology with networks of clinicians, suppliers, and installers to deliver personalized home modifications and environmental supports nationwide. We're looking for an Occupational Therapist who is passionate about helping older adults and individuals with disabilities thrive safely and independently at home. A successful candidate is highly empathetic, can juggle the needs of our clients, programs and business, and is eager to make a lasting impact. We're a tech-first team so comfort working with new systems and tools will be an advantage. Also a huge plus if home modification work is part of your experience.

Role Highlights
  • Anticipated Start Date: 8/1/26
  • Compensation: $120 per Evaluation
  • Employment Type: Contracted, minimum of 10 hours of weekly availability
  • Location: In-Home Visits
Key Responsibilities
  • Conduct in-home assessments and functional evaluations
  • Deliver personalized, evidence-based recommendations for home modifications
  • Educate clients and caregivers on fall prevention and safe mobility techniques
  • Engage clients and families in collaborative care planning
  • Work closely with our Director of Clinical Operations to share findings and support process improvement initiatives
  • Maintain accurate and timely documentation through our electronic system
Qualifications
  • Degree in Occupational Therapy from an accredited program
  • Active, unencumbered OT license
  • 2+ years of clinical experience
  • Experience in home health, aging in place, or disability services preferred
  • Strong interpersonal and clinical reasoning skills
  • Reliable transportation and valid driver's license
  • Proficiency with technology and digital documentation
  • Ability to work independently while collaborating with a remote team
More About Compensation
  • Contractors are paid on a monthly basis, within 2 weeks of the previous month's end.
  • We pay per completed evaluation - this includes:
    • Travel to/from client's home
    • The evaluation itself
    • Documentation
  • If needed, we will pay additionally for extended travel - we will always discuss and approve ahead of time
