What You'll Do
  • Perform comprehensive nursing assessments and skilled visits
  • Complete Start of Care (SOC), Resumption of Care (ROC), Recertification, and Discharge assessments
  • Develop and implement individualized plans of care
  • Provide skilled nursing services, including medication management, wound care, disease management, and patient education
  • Monitor patient progress and communicate changes with physicians and the interdisciplinary care team
  • Coordinate care with therapists, social workers, and home health aides
  • Complete timely and accurate clinical documentation using WellSky
  • Educate patients and caregivers to promote safety, independence, and improved health outcomes

Qualifications
  • Active Virginia Registered Nurse (RN) License Required
  • Valid Driver's License and Reliable Transportation
  • Strong Clinical Assessment, Critical Thinking, and Documentation Skills
  • Home Health Experience Preferred but Not Required
  • OASIS Experience Preferred
  • Experience with WellSky Home Health is a plus
