Serving Deschutes, Crook, and Jefferson counties, our dedicated team of over 200 employees and 200 volunteers delivers compassionate, patient-centered care across a 10,000-square-mile area. Additionally, Hospice House in Bend, Oregon, is a specialty hospital for inpatient hospice care, creating an oasis of tranquility and comfort for patients and their families. Compensation: $42.00-60.53 per hour, based on experience.
Status: Full Time (hourly, non-exempt) 40 hours per week.
Hours: 8:00 AM to 5:00 PM, Monday-Friday. Work outside of these hours/days may be required.
Location: This role involves local/regional travel throughout Central Oregon via personal vehicle during the workday, including visits to patients' homes and facilities.

Summary/Objective: The Support Registered Nurse plans, coordinates, implements, and evaluates hospice services and will cross-train in Home health. The Support RN is experienced in nursing, with an emphasis on community health education. The professional nurse draws on the resources of the organization & community to provide services that meet the needs of individuals and families in their homes. The Support Registered Nurse is responsible for cost-efficient and effective patient care with quality outcomes.
  • Complete an initial comprehensive (if HH includes OASIS) and accurate assessment of patient and family to determine home health needs. Provide a complete physical assessment and history of previous illness (es) and uses the health assessment data to determine appropriate diagnosis and need for other disciplines and resources.
  • Develop an individualized care plan, which establishes goals based on nursing diagnosis and incorporates therapeutic, preventive, and rehabilitative nursing actions. Include the patient and the family in the planning process.
  • Provide nursing procedures and treatments and/or end of life comfort procedure per plan of care. Administer medications and treatments as prescribed by the physician.
  • Complete and update the ongoing assessments, and care plan as patient's status changes.
  • Ensure that arrangements for equipment and other necessary items and services are available.
  • Provide health care instructions and education to the patient and or family as appropriate per assessment plan and patients' caregivers' ability to learn. Adapt teaching styles to meet patient's needs and family's needs.
  • Coordinate patient care with patient's case manager. Provide verbal or email updates as necessary.
  • Develop, monitor, and update the HCA plan of care as necessary including completion of HCA supervisory visits no later than every 14 days.
  • Communicates with the physician regarding the patient's needs, reports, and changes in the patient's condition; obtains/receives physician's orders as required.
  • Communicate with all members of the healthcare team to promote coordinated, efficient care and document coordination of care with all involved with patient care.
  • Initial assessments are complete, accurate, and submitted within 48-72 hours of admit. Additionally, ongoing clinical documentation is complete, accurate, documented, and submitted within 24 hours of the patient visit. Complete required reports/documentation prior to paid time off.
  • Communicate with facility staff, providing written updates and orders as requested.
  • Home Health comprehensive/OASIS assessments are completed within five calendar days.
  • Participate in on-call duties as defined by the on-call policy.
  • When working after-hours (outside M-F 8am-5pm) follow the guidelines for making visits as listed in the on-call/weekend services policy and procedures.
  • Local/regional travel by personal vehicle during the working day, extending throughout Central Oregon.

Work Environment:
  • May be exposed to infections and contagious diseases. Contact with patients under wide variety of circumstances.
  • May be exposed to/occasionally exposed to patient elements. Subject to varying and unpredictable situations.
  • Handles emergency or crisis situations.
  • OSHA exposure category 1: Position includes tasks that involve exposure to blood, body fluids, tissues.
  • Required Personal Protective Equipment as conditions warrant:Gloves, gown, mask, mask with shield, goggles, shoe covers, bouffant cap.

Required Skills & Qualifications:
  • Unencumbered Oregon RN License.
  • Ability to pass a background test, including a Motor Vehicle Report (MVR), drug screen, and TB test.
  • Graduate of an accredited nursing school.
  • One to two years of recent medical care experience in an institutional setting is preferred. Ideal candidates will possess 1-2 years of home health or hospice experience.
  • Current automobile insurance is required.
  • A valid Oregon driver's license and reliable transportation.
  • Valid CPR Certification.
  • Strong communication and interpersonal skills.
  • Ability to work independently and collaboratively within an interdisciplinary team.
