We are seeking a motivated and compassionate Registered Nurse (RN) to join our home health and hospice team.
Position Summary

The RN provides skilled nursing care to adult patients in their homes; coordinates care with the interdisciplinary team and serves as a primary point of contact under the Director of Nursing. Responsibilities

  • Provide skilled nursing care to adult home health and hospice patients

  • Coordinate care with physicians, families, and interdisciplinary team members
  • Educate patients and caregivers on diagnoses, medications, and care plans
  • Maintain accurate and timely clinical documentation
  • Participate in on-call rotation and respond to PRN/emergency visits as needed
Qualifications

  • Current Texas RN license (required)
