Job description

Job Title: RN Case Manager
Reports to: Director of Re-Entry
Classification: Part-time, exempt
Mission:
The Next Door Recovery provides a continuum of evidence-based substance abuse and mental health services for women in environment of faith and healing to restore hope and a lifetime of recovery.
Summary:
RN case manager (CM) is responsible for planning, implementing, and coordinating health care resources/services to meet the physical and mental health needs of women at TNDR Chattanooga Re-Entry Center, preparing them for successful reentry into the community.
Primary Responsibilities and Duties include the following:
  • Initial admission assessment and medication reconciliation
  • Manage resident medication processes including organization, storage, receipt, and prescriptions
  • Provide oversight of the daily dispensing of medication.
  • Coordinate medication prescriptions and refills, communicate with medical providers,
    TNDR residents, and TNDR staff.
  • Collaborate and connect residents with community health care providers and pharmacies.
  • Participate in weekly treatment team meetings.
  • Ensure timely and professional documentation per TNDR guidelines.
  • Provide nursing triage to residents with medical or mental health concerns.
  • Administer basic first aid to residents if indicated.
  • Provide health education to residents.
  • Provide consultation to TNDR staff regarding resident's health conditions and/or medications.
  • Additional responsibilities as assigned.

Education and Experience:
  • Bachelor's of Science or Associate's degree in Registered Nursing
  • Minimum of 1 year experience as a Registered Nurse
  • Valid unrestricted Tennessee Registered Nursing license
  • Knowledge of psychiatric disorders and alcohol and substance abuse
  • Knowledge of community resources and health care service delivery across a continuum of care
