Nurse Next Door | Ontario

Position Overview

Nurse Next Door is seeking a Registered Nurse (RN) or Registered Practical Nurse (RPN) to support clients and their families in maintaining independence through education, collaboration, and hands-on care. Working closely with the Care Designer and interdisciplinary care team, you will deliver safe, innovative, and client-centered care tailored to each client’s goals, environment, and needs.

Compensation
  • Hourly Rate: $31.68 per hour

  • Vacation Pay: 4% vacation pay added to hourly earnings

This position’s compensation falls within the legislated pay transparency requirements.

  • Scheduling: Flexibility required for client visits and scheduling needs

Key Responsibilities
  • Conduct comprehensive health assessments for clients and their families or caregivers

  • Identify care needs and develop individualized, goal-oriented care plans

  • Provide health education and hands-on training related to:

    • Personal care

    • Medication management

    • Mobility and safety

  • Support clients and families in navigating the healthcare system and accessing community resources

  • Monitor, evaluate, and adjust care plans to ensure best practices and client safety

  • Maintain accurate, timely, and thorough documentation

  • Communicate effectively with clients, families, and the interdisciplinary care team

  • Promote safe, client- and family-centered care within each individual’s home environment

  • Use mobile and electronic applications for documentation and communication (e.g., AlayaCare, phone, email)

Reporting & Collaboration
  • Reports to: Care Designer

  • Collaborates with: Care Designers and interdisciplinary care team

Key Performance Indicators
  • Attendance and reliability

  • Compliance with Attendance Monitoring System (AlayaCare)

  • Quality of client care and documentation

Qualifications & Requirements
  • Current registration in good standing with the College of Nurses of Ontario (CNO) as an RN or RPN

  • 3–5 years of nursing experience in a clinical or community setting (home care experience is an asset)

  • Demonstrated excellence in health teaching and client-centered communication

  • Two professional references

  • Valid CPR and First Aid Certification

  • Valid driver’s license and access to a reliable vehicle

  • Ability to travel independently throughout Ontario

  • Current Criminal Record Check with Vulnerable Sector Screening

  • Strong written and verbal communication skills in English

  • Comfort using mobile technology and electronic documentation systems
